Plot

Dr. Feinstone breaks free from the asylum where he has languished since his violent rampage triggered by his wife’s infidelity. Seeking a fresh start, he relocates to a tranquil Midwestern town, adopting a new identity as Dr. 'Caine' and opening a dental practice. Just as he begins to rebuild his life, he discovers his new girlfriend in an embrace with another man. The sting of betrayal catapults him back into a spiral of insanity, leading to gruesome acts of violence against his unsuspecting patients as he unleashes his rage once more.
