Sean finds himself taken aback and unsettled by the revelation of Elliot and J.D.'s past relationship. However, his frustration masks his true feelings about an impending six-month trip, as he fears Elliot's reaction. Meanwhile, Dr. Cox reluctantly sets aside his ego to seek assistance as he vies for the role of Residency Director at the hospital. Additionally, Carla takes on an extra shift as a surgical nurse, which leads to a day of working alongside Turk, much to his irritation.
