While on a train journey, Marian finds herself caught in a crash, trapped within the debris. Despite her inability to exit, she can navigate the carriage to aid the injured, with support from fellow passenger Dr. Nick Burnett, who reveals he is the new locum for the Royal. Meanwhile, the hospital faces mounting pressure in the casualty department as the first wave of crash victims starts to pour in, exacerbated by a staff shortage.
