Carter faces a tough first day as he loses his initial patient. Tensions rise between Susan and Kerry Weaver when Weaver insists that Susan must obtain approval for all her procedures. Meanwhile, Chloe once again leaves Little Susie behind to pursue a profitable venture in the flea market industry. Carol completes her paramedic recertification by attending to an extremely overweight and sluggish man. Shep makes advances towards Carol. Jeanie decides to end her relationship with Benton. Wendy interviews various ER staff for an article about Dr. Greene, while Dr. Ross tends to a four-year-old boy with AIDS.
