The competition for the Chief position heats up as Derek, Burke, Addison, and Mark all vie for Richard's approval, eagerly stepping in to lend a hand wherever they can. Meanwhile, the Denny Duquette Memorial Clinic opens its doors but only attracts a single patient, leaving Izzie disillusioned about the legacy she invested in and prompting her to attempt to draw patients away from the ER. After returning from Las Vegas, Callie and George shock everyone with news of their marriage, but Izzie is unsupportive, trying to convince George he’s made a mistake. In a significant turn, Meredith's mother, Ellis Grey, who suffers from Alzheimer's, suddenly becomes lucid for a day, expressing her disdain for Meredith's current life and her resentment towards Derek. As Ellis faces heart surgery, Meredith makes the difficult decision to schedule the operation against her mother’s wishes. Richard discovers that Ellis still has feelings for him, but their conversation ends abruptly, erasing her memory before Meredith can express
